Patient Forms
If you are a new patient to our office, please visit our Patient Forms page to download our new patient bundle with forms that will need to be filled out when you arrive at our office. Printing them, filling them out and bringing them with you will allow us to attend to your medical needs more quickly than completing them on your arrival. Payment Options
Balance payment is expected at the time services are performed, and a 50% deposit is required for all surgical treatment upon booking your appointment. For your convenience, we accept Visa, Mastercard, Debit (Interac), and Dental Card. Should you choose to pay cash toward your billing, please note that we do not keep a float on hand and cannot give change. Please feel free to contact our office should you have any questions regarding payments before your appointment day.
Dental Insurance
Our office does not direct bill to dental insurance plans; however, we will assist you in submitting the paperwork on your behalf to help ensure more expedited processing and reimbursement. We are also happy to submit any estimates to your insurance for you for your treatment plan. Your insurance plan will send all responses to you, so should you have any questions once you receive your pre-determination of benefits, we can certainly help to decipher it for you. Your insurance is an agreement between you, your insurer and your employer (where applicable).
